Ortho McNeil Issues Warning for Ortho Evra Birth Control Patch - Blood Clot Danger

NOV. 10 8:06 P.M. ET - Ortho McNeil the makers of the popular Ortho Evra Birth Control Patch warned millions of women Thursday that the patch exposes them to significantly higher doses of hormones and may put them at greater risk for blood clots and other serious side effects than previously disclosed. The warning from Johnson and Johnson subsidiary Ortho McNeil, makers of Ortho Evra, says women using the patch will be exposed to about 60 percent more estrogen than those using typical birth control pills because hormones from patches get into the bloodstream and are removed from the body differently than those from pills. Thursday's warning comes four months after The Associated Press reported in July that patch users die and suffer blood clots at a rate three times higher than women taking the pill.
Citing federal death and injury reports, the AP also found that about a dozen women, most in their late teens and early 20s, died in 2004 from blood clots believed to be related to the birth-control patch, and dozens more survived strokes and other clot-related problems. Ortho McNeil spokesman Michael Beckerich was not immediately available for comment about Thursday's warning, which was approved by the Food and Drug Administration. More than 4 million women have used the patch since it went on sale in 2002. Several lawsuits have been filed by families of women who died or suffered blood clots while using the patch, and lawyers said more are planned. Documents released to attorneys as a result of that litigation show Ortho McNeil has been analyzing the FDA's death and injury reports, creating its own charts that document a higher rate of blood clots and deaths in association with the patch than with the pill. In addition, an internal Ortho McNeil memo shows that the company refused, in 2003, to fund a study comparing its Ortho Evra patch to its Ortho-Cyclen pill because of concerns there was "too high a chance that study may not produce a positive result for Evra" and there was a "risk that Ortho Evra may be the same or worse than Ortho-Cyclen." Last week, in response to AP questions about the Ortho McNeil memo, Beckerich said in a written statement that "decisions to fund studies are based upon scientific merit." Beckerich said Ortho McNeil is conducting its own epidemiological study "designed with input from the FDA and similar to those previously conducted with the Pill." Although the patch and most birth-control pills contain the same amounts of estrogen, new published studies show that women using the patch absorb about 50 percent more estrogen than with the pill, said Dr. Leslie Miller, an associate professor of obstetrics and gynecology at the University of Washington. When women take the pill, the medication is absorbed into the bloodstream through the digestive tract. In the process, about half of the estrogen dose is lost. Hormone levels in women on the pill are highest one or two hours after taking it, Miller said. Twelve hours later, estrogen levels are quite low, meaning the body is not exposed to high levels of estrogen 24 hours a day. But the patch causes higher estrogen levels since delivery of medication continues all day. Those elevated levels but may be high enough to increase some women's risk of blood clots, Miller said. "If the patch is delivering too much estrogen, then it may need to be redesigned," Miller said. "Women should not just take off their patch, they risk pregnancy. If they are worried and want to change off the patch they can wait to get something else." Jennifer Cowperthwaite, 26, of Broad Brook, Conn., still suffers breathing problems after a blood clot reached her lungs two years ago while using the patch. She said the warnings were long overdue. "I wish I had known," she said. "It's quite likely I would never have used it." Erika Klein's sister Kathleen Thoren died a year ago from blood clots in her brain that the coroner said were brought on by Ortho Evra. She said women deserve to be informed when making birth control decisions. "Women have a right to know the true risks and make their decisions based on that information," she said. "No one should have to go through what my sister went through

so far been doing ok on the patch. except for when i'm a day or two late, i get severly sick. after 24 hrs. i'm ok again. but mean while i can't stop vomiting. is it still ok to use?
Moderator: I must qualify my answer by saying I am not an M.D., but good heavens, why would you want to stay on a birth control method that makes you so ill? Even if it is safe for you, which I would question, you can certainly find contraceptive methods that do not exact such a price! I've been on the patch for 3 years and at least twice a month I get really bad leg pains, so bad I can't walk. my doc. told me it was from the patch and I had to get on something diferent. They think it might of been blood clots. I wish I would of known I would not of got on it. I would of picked something different.

I've been using the patch for about 6 months. It's been the best birth control I've ever taken. I haven't had any side effects, yet. I've taken many different types of birth control pills and I've always gotten terrilby ill. I would suffer headaches, vomiting, mood swings, fatigue, I didn't have any sex drive. I love the patch. On the other hand, how long had these other women been using the patch before they had any symptoms or noticed problems.
Moderator: At least one of the women involved had used the patch for a very short time. All hormonal birth control carries some degree of risk. With the patch working so well for you it does make the decision difficult to decide on your own. You really should discuss this with your physician as many have stopped prescribing the patch now that there is questions about whether it was adequately tested.

I am 17 years old.I've been using the patch since late June. I thought the patch was one of the best birth control alternatives out there. My partner warned me that it wasnt good for me.Plus, I did here about a female who died a year before in a train station.She was later found to have suffered from a bloodclot due to the patch.I need to find a better prescription before i can just stop using the patch.I cant risk pregnancy again.I am currently suffering side affects from its usage.Weightgain,mood changes,nasuea and tiredness are among the complications.

Any word on whether the same risks are true of the NuvaRing?
Moderator: My personal opinion is that Nuva Ring needs more research too as it also provides direct and continuous release of hormones like the Patch. This method of delivery just hasn't been studies as thoroughly as the pill which is a cyclic dosage - it is gone from your bloodstream within 22 hours. The good news is the Ring uses a much lower dosage of estrogen than the patch and should be safer. On the other hand, since absorption takes place through mucos membranes instead of the skin your exposure might be higher. We need to know more!

I have been using the patch for about a year now. I didn't really have any problems until recently. The last 4 months, I have had back pain, headaches, and terrible mood swings the week of my period. The patch was wonderful while I had it on, but the day I removed the patch, watch out... the symptoms would cause pain and my mood would swing to an all time low. I cried non stop from the time my patch would come off until my period began.

my daughter is using the patch now, she has started having nose bleeds, she has never had nose bleeds, she is 16 years old, she has headaces, i saw the news on tv on wednesday warning about the ortha evra patch, i immediately had her to remove the patch, i truly believe this patch is causing my daughter these problems, i wish i have known this when she started using this product 7 months ago, she has even stated having a lot of mood swings a lot of weight gain.

My wife was on a high dose regimen of Birth Control Pills to control her menopause. She died in my arms 7 days later of massive blood clots in her legs that traveled to her lungs. (Pulmonary Embolism-PE due to Deep Vein Thrombosis-DVT). She told the docs several times about the pain in her calves, but they said it was just a pulled tendon. They gave her ice, motrin then vicodin for muscle pain. Finally, she told the docs that she could hardly breathe and felt like she was dying, and they said it was only a Panic Attack and gave her anxiety medication. On Saturday, she screamed, "Steve!", I ran upstairs to find her choking on the bed with her eyes rolled back. I slapped her and shook her (lovingly) and she stopped, looked at me for a few seconds, and then through tears said, "I think I am dying, take care of the children". Then she died in my arms.:(
WARN THE WOMEN AND YOUR lOVED ONES...I UNDERSTAND THIS IS COMMON IN OUR COUNTRY. She was only 48, sweet and lovely.

I am 26 years old, I have used the nuvaring and it didnt work out. I alos used the pill several times but that didnt work out as well' but as soon as the patch was made available to me Iused it and I have had no problems with it, but if they're saying that it's causing problems then I am going to stop using it.

I am 17 and i a have been on the patch for 2 years now, i think it is great. I have had no side effects from using them. They are great for the poeple that do not have time to take the pill every day thanks to the fact that you only have to change your patch once a week. I hope that everyone gets the same experience as i have had.

I used Ortho Evra just short of 2 years and it was fantastic for the first few months of use. I liked using it once a week and it made my periods lighter and shorter and cramp free. But within a couple months my blood pressure was higher and I was getting headaches (sometimes 2 or 3 a month) that would disable me for no less than 10 hours at a time...always on the left side of my head and completely crippling. There was sensitivity to light and blurred vision...scariest of all there was facial droop almost everytime. I am a paramedic and trained to recognize signs of stroke or TIA. I just didn't have medical insurance so I suffered through it everytime. I could have died! The patch also caused me to gain 40 pounds- which, due to the weight limit of 198, may have saved my life since I was taken off of Ortho Evra and subsequently began using the Nuva Ring which I have been using for 8 months now with no adverse side effects. My blood pressure has come down and I have had not ONE headache (or mini stroke perhaps I'll never know) since then. The right method of BC is different for everyone, but the patch is not for me. The risk of death is not worth it!

i am 24 yrs old and am using the patch since six months,for me it was trhe best thing ever, i have absolutely no problems but if it is supposed to be dangerous ,i will stop using it.whta are the other safe bc measures
Modertor: Please ask your doctor what might be best for you. All forms of hormonal birth control carry risk. The problem with the patch is that women were subjected to higher levels of risk without this risk being disclosed. It is believed that the manufacturers and the FDA knew of the increased risk and did nothing concerning the labeling of the product or educating of doctors to protect women. Millions of women still use the patch.
Safer methods of birth control include Cervical Caps, IUDs (not the ones from the 70-80s - the new ones are entirely different), sponges, condoms and natural birth control. Browse the BirthControl.com website to read about your alternatives!

I've been using the birth control patch for over 2 years now and suddenly a few months ago it started making me sick to my stomach again and I've been spotting a few days before my period is due. Do you think I need to see a doctor regarding these new symptoms

I am 27 years old and was using the patch for 2 years until I had a severe cramp in my right calf. I had scan done and it turned out that I had a blood clot. While recovering from the clot I had to do so much work to take care of myself by taking blood thinners for 2 months, blood work every month or week, shots twice a day, etc. Finally after 6 months I have been able to go off of the medication(coumadin) and have been told that I cannot use birth control except for the Merena IUD. I have had it for a week today and have had the most excruciating cramps. Is this normal I expected cramping but nothing this painful. Has anyone else experience these problems with the Merena DO NOT USE THE PATCH, NOW I AM HIGH RISK!

I've been taking the patch for about 4 cycles now and haven't had any of the adverse side effects that I've experienced in the past with the pill. With the pill, I gained weight, my face broke out, I had headaches all the time, and I was VERY moody. With the patch, I haven't suffered any of this. I have three children and it's sometimes difficult to remember, "Did I take my pill today" The patch has been a very convienent method for me and my husband--it's going to be a difficult decision on whether or not to keep using them or to try something else. When I initially spoke to my Dr. about using the patch, he said that this form of BC would suit me because of it's continous release of hormones--with the pill you have highs and lows and these "peaks and valleys" cause wieght fluxations and moodiness in women. It would be interesting to see if the 17 women that have been cited as dying from the complications of the patch were smokers or lived/worked in an environment of a smoker Additionally, I thought that hormonal birth control wasn't recommended for women 35 because of their increased chance for blood clots I think that anyone who uses this method needs to base thier decision on a personal basis--because the patch DOES work for some people...it's going to be hard to decided. I've scheduled an appointment with my Dr. to talk to him about it!

I have been on the patch for a year now and i don't like it i'm starting to have chest pains real bad mood swings, and breathing problems Sometimes I have to take deep breaths because it feel like i'm going to stop breathing. If I know this was going to happened I would of never used it

I've been using the patch for 3 months straight. My OB/GYN instructed me to use it this way for acne control. I've noticed my breast getting bigger and are tender. At times they feel like when I used to breastfeed my kids. Should I be concerned
Moderator: I would mention it to your doctor. In a perfect situation we wouldn't experience any physical or emotional changes from our birth control choices. It may not be a concern, but it is at least unnecessary.

I've been using that patch now for a year. At first, I thought it was the greatest thing because BCP's made me nausious all the time. But for the last 6 months I've been noticing that I've been having chest pains and severe PMS.

I've been on the patch two years. Everything was great. I thought it was the best thing. I really suck at taking a pill everyday, there's been times when I forgot to change my patch! Anyways, I went off the patch a few months ago just cuz I was too busy to see my doctor and renew my perscription. That was when I began to have problems. I ended up having my period for three months! I had to go back on the patch to make it stop. Now when i get my period it's very heavy and I get headaches. I now have to go back to my doctor to fix this problem. My main worry is if I can have more children. The patch suck!

I'm 30 years old and i've been using the patch for almost 4 years. It's been okay for me, except that i've gained weight and get mood swings, but i will be searching for a new birth control soon. I didn't know it caused blood clots, until I heard it in the news. I wish they would of told us about this sooner.

I've only used the patch for 1 month and I went off it for about a month. I did not go off it for any reason but to decide if it was the right method for me. 4 weeks after I was off the patch I started to get alot of pregnancy symptoms (but I am not pregnant)and now I have lower back pain and in the abdomen and now alot of pain in my lower left leg. Now I know what to tell my Dr. to look for so I don't become a statistic. I want to thank everyone and their concerns and coments. I want to give my condolences to the gentleman who lost his young wife. It is a huge eye opener to the birth control. I don't know if I want to even take any kind of birth control until I have done my homework because even the medical field that you trust is uninformed. Thank You

I have found this article a little too late. I have been on this patch for a little more than 5 months, I almost died on January 16, 2006 from blood clots in both lungs. My doctor did not discuss the dangers of the patch with me. I am now taking control of my life, no more pills, patch, cream ect

I just started with my bith control patch and I'm 18. My mother looked up this article on the internet and informed me of the affects of the patch. She told me to take it off immediately. She looked at the area where the patch was removed. She told me there were bumps there after i had told her that the patch was irritating because it made my back itch. She told me I didn't want a birth control so i didn't need one. I told the doctors I wasn't going to have sex but they told me I still had to choose what birth control I wanted to use and they informed me that every birth control has an affect. The only information given to me is they have blood clogs andthey may make you gain weight. Well, there sure was a lot they forget to mention or did that information just jump and haul tail down the road when I came into the room to discuss a birth control I never wanted in the first place. This article probably saved my life. My day to change my patch was on Fridays and today is Thursday. I haven't used the patch for a full week. I'm glad I found out this information early. Thank God.

I am almost 21 years old and I have been on the patch since it came out. I don't think I have any side effects but on the other hand I do sometimes get sharp pains in my chest and my left arm. I'm not sure how I feel about the patch anymore. I don't even know if I'm going to get off of it. I have noticed a slight weight gain and my breasts have enlarged. However with all of this news about it I am going to go and talk to my OBGYN.

i went on the patch after my daughter was born in 2003. i got really bad headaches. they would last for 2 day. i went off the patch in april 2005. got pg in may and just had my baby in feb. i'm glad i read this cause i'm going back on birthcontrol in april. thank you for the info.

I understand that women need to be careful, but I would like to advise them not to become paranoid. Yes the patch gives a higher dose of horomones which can lead to a higher risk of blood clots, but you have to understand- this is a higher dose of horomones compared to the BC out on the market TODAY. Compared to the BC that our mothers used, this is the same if not a lower dose of horomones. So if you mom or aunts used BC in the past without any problems or blood clots, you should be fine. This is not to say that you should not still pay attention to what your body is telling you.
I started the patch a year ago and yes, at first I had side effects, but they go away. I got the back cramps, the mood change, the nausia to certian foods and yes, weight gain. These are the same side effects you can get with ANY BC. last summer I lost the weight and it hasnt come back. The cramps and backaches go away too, just like my doctor said they would. So if you just started the patch, dont give up on it- these effects can last you up to 4 months. So dont panic.
As I previously stated, I am not telling you to ignore your body. If you are having severe leg pain, debilitating headaches and throwing up each month- the patch is probabily not for you. Every woman is different. Also, remember, please do not smoke if you are on the patch. 98 of stroke victims that come into the ER are smokers. This risk added to the horomones is russian roulet.
For the women who have headaches as their main side-effect, look into the other possible causes in your life. Headaches can be caused by millions of things. Their coinciding with your patch could be a coincidence. Do some research prior to panicing and throwing out all of yoru patches.
One last note before I go, I STRONGLY ENCOURAGE all women on any sort of BC to look up the warning signs of a blood clot. Then, don't get paranoid, but pay attention to your body and keep an eye out for those signs. The signs of a clot are pretty obvious. My heart breaks for all of those women who have been hurt or killed by BC related blook clots. IF you are on BC and are having severe symptoms, take into consideration that your doctor is not right 100 of the time and you should maybe get a second, third or fourth opinion. It's just not worth it not to.
God Bless all of the women out there. Be aware of what your body is telling you but feel free to live your life and enjoy a safe and healthy sex life.
